Let's be honest, we all make mistakes. I have too many times while playing HS that I care to admit. What is your worst move ever that caused you to lose the game when, had you played correctly, you'd have won (not just missed lethal or due to bad RNG)?
Just a few games ago I was against a hunter and I was playing freeze mage, I had lethal but he had a lorewalker cho out from his piloted shredder. So I kinda used ice block first instead of my burn causing him to get ice block then spam well played, and that lost me the game lol.

using Lord Jaraxxus or Alexstrasza way too early to try to get ahead with powerful minons, and it only makes me lose. much better reserved as a heal when almost dead.
another derp move I make more often than I like to admit is playing Twilight Drake or Mountain Giant after playing other cards that are also in my hand.
I was playing hunter and used my call companion card and got the 4/2 boar. Well, I hit end turn before attacking with it. Next round it was backstabbed and yeah, I walked away.
Accidently dragged a frostbolt to my face instead of my opponents minion on my phone. It was against a priest and at the end of the match I could have taken him out if I'd had just 3 more damage....
another- playing as warrior on my phone and not noticing I had a weapon equipped that could have saved me on a play (on mobile the cards cover your hero when you look at them)
killing off my own baron geddon (1hp) with the whirlwind effect of my deathsbite to miss lethal...

I've missed attacks both from charge minions and from minions already on the board. Not the usual "Bluegil came out of shredder" ones either, i played an actual Arcane Golem from my hand and passed the turn...
my biggest mistake is playing when i'm very tired. usually i lay in my bed after work and play HS on my phone and i almost every second match make a derp move. example, over damage Frothing Berserkers,coin a spell when using pyromancer, coin for nothing etc

I used a Circle of Healing on the first round thinking it was my The Coin...

In no particular order:
I've cast a spell after killing a Piloted Shredder before the animation completed only to find out I just have my opponent a spell off Lorewalker Cho.
I've played Draconic Crusher before going face to get an opponent under 15 the same turn in arena.
I've played The Coin out when I didn't need to or it doesn't help because of fuzzy math.
I've cleared minions before playing Unleash the Hounds and Sea Giant when it was relevant in both instances.
I've played around secrets really poorly in some instances because of terrible assumptions I've made.
I'm sure I've missed lethal on several instances.

Play Twilight Drake after playing tons of other cards because I didn't planned my turn...
I killed my own Frothing in the midst of a comeback otk that I would have won had i not accidentally killed my frothing.

securing the win by playing loatheb against a druid who almost certainly will drop combo to kill me next turn
and then i play the loatheb
30secs later with a broken mouse beside me i concede in despair...

Just played a CW as Handlock.
I played the matchup really well, saved an owl for Sylvanas and Siphon Soul for Ysera.
Played around Brawl all the way to the end, which he had to use for a marginal return.
Then I over extended into second brawl, lost the Brawl 1/6 and Jaraxxus doesn't save you from Grom from hand :(.
Why did I over extend when I didn't have to? Why did he have to have the second copy, and then win the 16.66% roll?

As a a rogue summoning the 2/1 charge pirate use my weapon it breaks.... then i cant trade
Also i was about to turn 3 sprint with coin prep but used prep first DX
installing HearthStone.
