It's rare to ever see a successful friend request. I only do it myself to find people for the spectate quest. Anyway, when do you accept friend request and when do you friend request?
I always accept friend requests. If someone wants to baby rage at me then it means I'm playing well... anyway, it's fun to wind them up even more! Angry Shamans are the best!! :)
If they baby rage, I just ignore them, and some people actually don't rage after they lose and I tend to leave them on my friends list. I only request people I know in real life, that means not really great numbers ;)
I virtually never accept friend requests anymore since they in close to 100% of the cases are just pre-pubescent boys BabyRaging. I very rarely send friend requests myself either. If I do it's generally to ask about the opponents deck or something similar.
I accept if they had a positive attitude during the game. I also like it if the other player has perfected the art of the emote, as opposed to the usual BM behavior. Some players really know how to time their emotes, which I interpret as a good sense of humor. My friends list is filled with rl friends who play Hearthstone so I don't care for adding randoms. Someone who comes across as a nice or funny person is someone I'd like to befriend and chat or test decks with.

- My opp played so bad, that i want to explain/teach what went wrong, and point some errors that made him lose the game badly. - The game was sufficiently awesome to "award" it. And ask for weird decisions on specific cards.
But i don't accept friend requests like in 99.9% of the times. Specially after one sided games (winning or losing)
Honestly, they get more pissed if you don't add them. Letting them rage at you is letting them vent their anger, which will cool them down in the long run.
I tend to send friend requests after a close game or if I've learned something from the game. Examples
1 - N'zoth Paladin mirror that I lost. It was a great game and I had no problem in losing. Just wanted to compliment my opponent and thank him for a great game
2 - Marginally beat a Priest mirror. Same reason, the dude in question had some great combos (can't remember off-hand) so wanted to thank him for a pleasant and fun match.
I've not had any friend requests sent to me yet but I imagine I'd refuse if there was emote spam or I suspected my opponent was intentionally roping.
I pretty much always accept since I either 1) Get to talk to a new potential friend about a game I enjoy or 2) Get to laugh at some rage. That seems like a win/win to me.
As for sending, I send after close, fun, or notable games.
I occasionally add people after a match even if I lose. If people add me, 99% of the time I'll add them back. It's rare that I get hate messages, and when I do, the person usually says what they need to say and unfriends me. Most of the people who add me though are friendly, and if they aren't, I don't waste my time dwelling on their messages or responding to them. I wouldn't let a fear of hate messages deter you from adding people, if someone's hostile towards you, chances are that they'll unfriend you soon after messaging you, and if they don't, you can always unfriend them yourself.
I tend to send requests after crazy games... for example, yesterday I was playing with my No-C'Thun C'Thun Shaman, after dropping him to 7hp I was clearly loosing, he had a full board thanks to his N'Zoth, he had lethal next turn, and i had nothing.
Out of desperation I topdeck Evolve, so I play my Twin Emperor who doesn't even duplicate, play Evolve, get Ragnaros, pass ad hit face instead of one of the 7 minions on the board.
I HAD to add him, just to say I was sorry. He didn't rage, we both regretted noone had recorded the game, laughed a bit and then parted ways...
I've never sent a request but I've had a lot sent to me. I usually wait about a week and then accept it, if they're still mad then, that's a personal issue that they should probably address.

JK but really I usually add people when something like this happens. Sometimes they add me if I get extremely lucky as well or exact draws every turn. TBH if I didn't accept friend requests good or bad I'd have no friends (in HS) cause I don't know anyone irl that plays the game. The people I've added are usually high skilled (High Legends every season)

Friends aren't necessary in Hearthstone, so I don't bother.
When I looked up "Ninjas" in Thesaurus.com, it said "Ninja's can't be found" Well played Ninjas, well played.
