Lorewise, Hearthstone is a card game playable in Azeroth. When you play Deathwing in Hearthstone, the idea isn't that Deathwing is swooping in and destroying everything. The idea is that an unseen adventurer in a tavern in Azeroth is playing a Deathwing card in a game.
