It really depends on what sort of deck your playing and what sort of deck/hero you're up against.
Unless you're playing a rush/beat down deck (attack all the time, leave the trades to your opponent) early damage isn't actually much of an advantage.
Druids, Rogues, Mages, and Paladins do not need to burn cards to remove him (although a Paladin might anyway). Priests with early Northshire will actually love you for playing him, and Warriors with the Armorsmith can at least mitigate the impacts.
Shaman can use Earth Shock on Leper Gnome and not only remove him from the board, but also silence his deathrattle...but that is burning a card. However, if a Shaman pops a Searing Totem, then that makes for a good trade.
When I'm playing aggro I've had a couple of times that I get them down to 2 before they stabilize and dropping this guy is the GG. I mean he only fits in aggro decks in my opinion but he gets a lot done for 1 mana. Aggro looks at card value differently from control and although classes can remove this with a hero ability and ends up +1 card thats still 4-6 damage with their turn two being used up, pretty good value in a rush deck.
for a turn 1 drop, yes leper gnome is the most versatile minion to be played on the first turn. of course there are better ones but its all very situational, though for every other situation, its marginal at best,
i'd only stick this in if you have no other better 1 drops and you absolutly need a 1 drop, otherwise its a filler for me
*Meh*, decent enough card for rush decks, guaranteed 2 damage for 1 mana, possibility for more, so it's efficient. In terms of board control, it's utterly useless, the amount of times that a leper gnome has managed to trade up against a minion of mine is... maybe 0?
Pretty good card, right? 4-6 damage + killing off a minion on a throwaway 1 drop.

I love him, because it's almost guaranteed hero damage, which works well with my aggro deck.
