It says "they attack instead", which probably means that it is only the hands that will do damage. It would thus deal 2x2x4 = 16 damage, when played with rush, and possibly 16 + 8 on the second turn.
Buffing this with Cariel will not buff the hands, so the damage output would actually be the same (untill the hands die).
