Patches if you like playing aggro decks. N'Zoth isn't really seeing any play in standard but is pretty strong in wild. Medivh is the played in one of the strongest decks in the meta right now, being control/freeze mage. Personally I would go with Medivh but it depends on your play style and what you have in your collection to build the decks that these legendaries fit in.
**Just realized that you can't craft Medivh. You have to unlock him from the 4th and final wing of Karazhan. Seems pretty expensive for one card so I would probably go with Patches in that case assuming you prefer Standard over Wild**
And people above me already said everything relevant, it's pretty much the matter of personal preference: if you play aggro, then Patches; if control, then N'Zoth. Both are good cards so you can't make a massive mistake regardless which one you craft.
If you reaaaaaally reaaaaaally like to play Aggro decks, Patches is a safe craft. Otherwise, N'Zoth allows you to create so many fun control decks it's nearly disgusting in the wild format.

If you have any of the powerful death rattle minions (i.e Tirion, Sylvanas or Aya if you're playing Jade) then N'Zoth is instantly worth it. As for patches, he's a really good way to boost tempo in the early game, especially in rogue if you've got a swashburgular. Both of them you can slot into most decks so just craft the one you feel is going to be the most useful,
Even if Nzoth and Patches are safe crafts (they cover many kind of decks), i would honestly wait for the next expansion, which can't be that far from release.

Mediv if you like spell based deck but you will have to buy the adventure instead of using dust to craft it
