So no, you don't need your opponent to have a <4 health minion each time and you will not take fatigue damage as long as your opponent does not disrupt your combo (Shadow Word: Death, Assassinate, and so on), which is very easy to do. Like I mentioned, this combo is in no where near good, but is not only able to sneak some wins through niche matchups, but seems like a very fun concept to work with (IMO at least).
Well in his defense, there's the soft synergy that you get a backup Mana Wyrm. Or maybe just the fact that if these two cards are run in the same deck (Mana Wyrm pre-nerf), it would just boost the power level of the deck significantly. No direct synergy between, but just having both of the cards in the same deck very annoying.
This gives the opponent this dilemma though, play no minions and die, or give your opponent a lot of minions and die harder. Of course there are always non-minion removals but the opponent probably won't have more than 2 left. And like you said, infinite DMHs and Rhinos.
Though I'm sure this deck will never ever take off, it's still a sweet interaction and idea to work with.

So you can accomplish:
1. DMH
2. 5/5 Akali
3. Deck is now: Akali, DMH

If I use a 0 cost Heal card for cycle, why wouldn't I go for Circle of Healing instead? Bar the fact that this can target Heroes as well.
