I was lucky enough to be able to use its effect 4 times. Played with Brann Bronzebeard on the board and then again with Ancient Brewmaster, a very very rare coincidence but it can be done.

Targeted battlecries (Fire Elemental) do not allow you to choose a different target when they activate a second time. This makes cards like Cabal Shadow Priest pretty useless to activate twice.
Randomly targeted battlecries (Bomb Lobber) will randomly select another target when they activate a second time.
Stat-buff battlecries (Twilight Drake) will double the stats they receive as a result of their effect. I don't have The Mistcaller, so since he has a fairly unique effect, I'm unsure the same applies to the cards in your deck and your hand when you play him.
Battlecries that summon minions (Dr. Boom, of course) will summon twice as many minions as usual.
Combos do not count as battlecries for the purposes of this mechanic, except for Perdition's Blade. It works like you would expect any battlecry to, even when its combo effect is active.
Crazed Alchemist and similar cards swap their target's stats twice, basically doing nothing.
Cards like Faceless Manipulator do not activate twice since the battlecry minion is removed. I'm not sure what they'd do anyway.
Cards with activation conditions that may be altered when they are played (Mind Control Tech) must meet the same minimum conditions necessary to activate at first when they trigger a second time. For instance, a MC tech played against four minions will only steal one, but when played against 5+ minions, it will steal two, since there will still be four minions on the board even after it activates once. Some cards are useless to activate twice for this reason, like Dark Iron Skulker.
Loatheb and Saboteur will successfully increase their respective costs by ten as opposed to the usual five.
Unearthed Raptor multiplies the effects of a deathrattle by two. If you use another raptor on the first. you can have 4x the original deathrattle, and, if you had unlimited raptors, you could multiply a deathrattle infinitely.
Joust battlecries still count as battlecries (despite the fact that they're weird), and, as such, they will activate twice. The same principle applies to the discover mechanic and other battlecries that draw cards

Maybe just lucky but I got it first try with this deck. The Twisting Nether almost screwed me but luckily I hadn't played my 48 C'thun yet. Didn't even have to use the Divine Spirit or Inner Fire.

I have a gold version of this card, thinking about dusting. Is it a viable card at this point or safe to dust? I do play shaman occasionally, and while I wanted this card forever, once I had it, I barely used it.

I subbed 1 Doomguard for an Enhance-o Mechano and it works pretty well, even better if you can do it with Brann. The extra divine shields and taunts just makes it more annoying for your opponent to deal with.

Didn't have the Paletress, Voljin, Twilight Guardians or Ysera. Replaced with 2 Big Game Hunters, Sylvannas, and Shadow Madness. Used the 4 mana crystal artifact to get out Sylvanas early and was able to take out one of his minions and then steal the big one. That really helped early game. It went smoothly after that.

Good deck, took a few tries but it worked without too much frustration. Try to keep the board clear until you can play Grim Patron. I threw in a Sea Giant for fun although I didn't need it.

Yay, this worked with a few changes! After a few tries of getting almost there but falling short, I swapped out some cards and finally got him. I took out 1 of each copy of the weapons, 1 Knife Juggler, 1 Ironbeak Owl, 1 Hauntedd Creeper, and added Arcane Shot, Freezing Trap, Snipe, Deadly Shot, and Gladiator's Longbow.

I thiink a 7/5 with stealth is a good option as you still get the extra damage and it won't die right away because of the stealth and can use it next turn. It'll also dodge a flamestrike. Or a 5/5 with divine shield and taunt, that way even with the smaller body, it can survive at least two hits unless silenced. Either way, I don't have any of these in my druid deck yet, wondering if its worth crafting one. I have all the other "essentials".

Hm, you are right. I am a casual player so I don't know all the values/worth very well yet. But still, would be nice to make the other choice better. Perhaps a choice between 5/10 with taunt or 5/5 with divine shield? Or a 7/5 with stealth? Eh, it doesn't matter I guess since they probably won't change it anyways.

Thanks, this was better than the priest decks I've tried. Beat on second try! I subbed a Lay on Hands and Eadric the Pure since I don't have Justicar and Hogger, but still worked pretty well.
