I think it'd be really cool if the cities were the battlefields. So each battlefield would have a very slight different effect. It would be very hard to balance and I'm not sure how it would work in a tournament though.
The death rattle is very easy to playaround by just never using the last charge. This is also a neutral card so couldnt a mage just ping enemies down to take them? I guess it would only work if the sword kills them, but the card text is already pretty long.
I really think there is no way to balance Arthas. If blizzard introduced him he would be his own class. You're better off just making Frostmourne a card on it's own.
I like the effects of them separately. I think frostmourne by itself is an incredibly strong card, but it would have to be whenever your hero kills an enemy minion. Otherwise you would just hold on to it forever and just trade minions all day.
I really love the lore behind the Old Gods (and everything Lovecraft). I tried to fit their methods in to a card idea of how their whispers would slowly creep up on you and lead to your death.
Similar to how in Ahn'Qiraj you would hear him say:
Death is close...
You are already dead.
Your courage will fail.
Your friends will abandon you.
You will betray your friends.
You will die.
You are weak.
Your heart will explode.
I'd love any kind of feedback or even any ideas you guys have for other old gods like Yogg-Saron

I think generally battlecries go through all things like this. Even minions that are immune to spells can be targeted by battlecries.
It would probably work if it were just a deathrattle, but I guess that could be silenced.

This is more of a heroic encounter I think. The idea is cool, but you're just going to go against a face hunter and be dead by turn 4.

The silence happens before the attack, similar to how freezing trap procs before any damage is done.
Charge and windfury minions would allow one attack before being silenced.

He was countering the point to why it needed to be immune to spells. It being untargettable makes it immune to sap and recycle.
