Shadow of Madness? Maybe?
Anyway, I have seen a couple of examples shown where someone could have shadow'ed a Bloodmage Thalnos for lethal, as well as several examples where Mechanical Whelps should have been stolen and traded off for the deathrattle.
In none of these circumstances was Shadow of Madness? offered.
In general, it seems as though the algorithm is too focused on spending the remaining mana of the turn efficiently. It is LESS likely to recognize lethal in general when the card that would give you lethal costs less than the amount of mana remaining.
Having said that, in general the programming on Zephyrs is spectacular. It is a particularly difficult programming concept to try to isolate the "best card" with so many different lines of evaluation for the definition of "best". Most of the time, this card serves you very well.
