I was playing Arena, Had a Sea Giant , and 2 more low mana minions on board, opponent on 16 Hp 1 card in hand, and I had a Big game hunter in hand...
So I thought of preparing for lethal next turn and putting down my BGH.. Totally forgot about my Sea Giant and pointed BGH on my Sea Giant.. one thing led to the other and I killed my Sea Giant...
I was just playing Priest in Arena. I had a Northshire Cleric and one other card (can't remember what) out. Long story short, I went on a healing/drawing spree with my Cleric and then ended my turn with 1 mana and a Lightwarden that had been in my hand since the turn started still in my hand. I then facepalmed myself, completely forgot, and did it again the next turn.
This isn't an in-game misplay or anything, but I was building a deck and spent about 15 minutes pondering whether or not I should put Archmage Antonidas in it before remembering I was building a Rogue deck.
During one arena, I had 2x 6/3s and 1x 5/3 out. Opponent dropped an Earth Elemental. I accidentally buffed the 5/3 instead with Abusive Sergeant. Had to run it and one 6/3 into the Elemental. Would've had lethal that turn if played right. Won nonetheless.

Not exactly as brilliant or multi-dimensional as some of the experiences on here, but I have Starfire'd myself in the face before. The look of horror on my face when it happened tho:|
I was entirely convinced that Whirlwind damaged heroes as well as minions and used two of them to finish off my opponent. I even said "Well Played". The shame turned into relief when I still won next turn, but it could've turned real bad.
This was really sad on my part and a bit unlucky, but it happened.
Enemy board had only Knife Juggler on it. My side of the board was clear. Played Leeroy Jenkins, not even considering the fact the 2 whelps summoned would proc the Knife Juggler. Both procs hit my Leeroy and he was dead. Needless to say I lost that game, but I learned. I still think it was unlucky for both procs to hit Leeroy...at least one of them could have hit my hero. :P
Playing (variant of) Zoolock. I'm on 10 hp, got Defender of Argus on board (3 attack) , opponent 7 hp,has some good damage minions that can kill me at his turn but no taunts on board. Draw Soulfire. "great got lethal, gg". Proceed to soulfire my defender of Argus because I somehow thought it was a Power Overwhelming. Instaconcede and crying in a corner.

Not devoted enough to spend money/lots of time , but still like being somewhat competitive
Peaked at rank 3 couple seasons ago, usually around ranks 10-5 at the end of seasons (unless it's been a busy month irl)
wild pyromancer and circle of healing derp moment :D shaman had 7 minions with 1 health nice combo but thats how I found out it heals first so he got to keep all 7 minions :D
This was a long time ago in arena so I don't remember specifics but:
- I was at 1 health and we were both just topdecking.
- I cleared his board then polymorphed some big dude. I had a full board of minions.
- I got really excited that I was going to win next turn and went for face
- I forgot to kill the sheep.
- I died to a goddamn sheep.
I friended the guy after the match and we had a good laugh about it :P
Hehe I made a similar blunder once, I'm on 5 HP with full board control, enemy in arena (Paladin) was topdecking and all he had out was a Silver-Hand Recruit.
My board is plenty big, but contains some pretty weak creatures due to rules having to be followed, hence I have to make a decision, kill the 1/1 dude or set up for lethal next turn.
Well my patience got the better of me and I swung all my creatures at his face. Naturally resulting in the Paladin topdecking BoK and my own demise.
Lesson learned, always count on a Paladin topdecking 4-damage (BoK, Truesilver, Argent Commander). And always clear the enemy board!
EDIT: One question to your story, if the Pala was on 1 HP how come you didn't just fireblast him to death? Or did you use that on face instead of Sheep to bring him to that 1 HP? :P
I think my worst is somehow thinking that deadly was +3 damage (seeing as it brings up wicked knife to 3) adding it to an assassins blade and then attacking into a +1/+1 ravenhold assassin. Yea, leaving it at 1 health while doing myself 8 damage, not helpful.
I did the same dumbass thing twice the other day. It involves Sylvanas and Deathwing. I imagine you can figure out the rest, however, I will elaborate.
I totally deserved the first. I had a Priest, who had MCed my Sylvanas, at 2 health, with like 5 damage on the board, so lethal. I was having a bad day and decided to BM. I still had 15 health and didn't even think about the Sylvanas, just that he had already used MC, so I should be safe. DUH! I was at 15 health, he top-decked an Argent Commander and won.
The second was a couple of games later. I think it was against a shaman. I don't remember what the life was like, but I was crushing him. I had played a Sylvanas, in case he played something nasty in an attempt at a comeback. He used faceless on it, and managed to protect his own. I played around it for a turn, but then he started to make said comeback and had like 6 minions on the field, while managing to get mine rather clear. The only thing I had to counter that was Deathwing. So I dropped him, having lost sight of the Sylvanas in the crowd. I immediately conceded and stopped playing for a while. I was obviously not thinking as clearly as I needed to be.
I was in a good position against a Hunter, who laid down a secret. Given that I had a few low-health minions on the board, I was certain it was an Explosive Trap. So planning out my next move, I said to myself "attack, pop the trap, then play your cards."
He took a while futzing around with the rest of his turn, and by the time it was my turn, I'd totally forgotten what I just said to myself. Think I drew something buff-ish like an Abusive Sergeant, got overexcited, player a couple of other weak minions, then attacked. Bang. Trap. My entire army decimated. He went on to win with 3 health left.

I was playing a paladin vs a hunter and I used my hero power when I already had 2 minions on the board. Man was there egg on my face...
*cries*
he said he was on 1 health not the paladin.
Let me change your mind
Hehe yeah just noticed that. Guess that's what happens when you post stuff right after you wake up in the morning, my bad.
I just handed my opponent in Arena a win when I Polymorphed my Jungle Panther instead of his Stormwind Champion...
Decks I'm using for Ranked play:
I think my worst is somehow thinking that deadly was +3 damage (seeing as it brings up wicked knife to 3) adding it to an assassins blade and then attacking into a +1/+1 ravenhold assassin. Yea, leaving it at 1 health while doing myself 8 damage, not helpful.
Anytime I play any combo for lethal, then realize after I've played a card or two that I don't have enough mana crystals for the entire combo.
