I think the concept is very cool. The only gripe I have about it are the classes that add minion types to it. That would be outside of hearthstone’s game design. Though I do enjoy the idea of a beast/dragon or a demon/dragon, it would be stronger in those respective classes if it was just a dragon and had another minor ability that is part of the class’s identity.

the reason why i had the 'this is also a beast" and "This is also a demon" is so that if the battlecry pulls stuff like demonfire or mark of ysarrj, you have this to interact with it.
I always hated when you played some random card generator, and it gave you something like "Beastial Wrath" in your control priest deck
EDIT: Changed the Shaman Form to read "Your spells that deal damage have overload (1)".
SOME OTHER IDEAS:
Rogue Form: Give it poisonous as well?
Mage Form: Originally didn't have the spell cost reduction on the mage one. I added it because I felt that its just a worse Azure Drake then, netting a random spell from a random class (instead of one you put in your deck)
I really like the transformations, but I'd definetly leave out the "add a random spell to your hand". Just seem very weird and out of place. Like you couldn't justify it's cost and shoehorned that effect at the end. And that "this becomes a beast/demon" thingy just doesn't fit the flavor at all. But I like the rest of the transformations and could see this working as a very fair card.
While I like the flavor, I think it's very random. Not only do yoy get a completly random spell, each form is very different and I!m not sure many decks would want to run a card with this much variance. I'd like it better if you could choose the class. Something like "Discover a spell. (From any class)" would be better in my opinion. Not only would you have some control over the spell you get but there would also be a nice tension : do I pick the best spell for later or the best class for my drake now ?
If you do that, I would also advise to simplify each form. Maybe having just one keyword on top of whichever stats seem best (5 Mana 4/4 with a keyword and discover a spell sounds very powerful). The thing is that players would have to remember the form for each class in order to take the best choice. Not all keywords are equal of course but I think the power levels would be closer this way.

I hear you, but i really want to not use discover. Its a fantastic mechanic, but it has been overdone, especially in custom cards. I kind of wanted this card to be a fun tempo/value card rather than an interactive/versatile card. I want the forms to be a bit powerful, with the cost being randomness and low stats.

I don't think its a problem that mage and shaman are similar though.
DRUID: I'd probably change Druid to something like "Deathrattle: Equip a 1/3 Dragon Tooth Dagger" that gives you armor every time you attack,
PRIEST: I'd probably take out the reborn on the priest one and give it Lifesteal and "Your Lifesteal effects also heal adjacent minions"
HUNTER: Eh, I kind of like the hunter one. There's a reason why Charge hasn't been printed in a while. its a really powerful effect and i think its kind of cool to bring it back on a card like this. And its really flavorful.

Some of the class forms are far too powerful, and some are far too weak. I think each one would need to be tweaked in some way, but the idea is cool.

It should have Rush for Hunter.
EDIT: Seeing that the Paladin and rogue had the lowest power level, I gave them a bit of a boost.
PALADIN FORM: Now has Divine Shield and "Adjacent minions have +1 Attack."
ROGUE FORM: Now has Stealth AND Poisonous.
