Blizzard has confirmed that they do look at custom cards, although I think in many of the cases of similar reveals it's just the players and the company having the same thought process. My two cards matched almost exactly, but one came too close to KoFT for it to be implemented and the other I didn't even publish.
Over a year before it was released, I made a card that was functionally identical to Forbidden Ancient (it was even a Druid card, only difference was name, art and rarity) and submitted it for an early competition. The image is gone but the explanation makes it clear.
My original, original version of "Spring Rabbit" (which you can't see anywhere) was kind of similar to Weasel Tunneler. It was a 1/1 with "Battlecry: Shuffle a copy of this minion into both player's decks."

JMAP94. The best way to love the artist is to love the art.
The wording is different as Underworld Cerberus's effect is also intended to work during your opponent's turn. Basically Foe Reaper that also works whenever it's attacked, not just when it attacks. For some reason, we all think of Hunter when we make this effect even when Hunter had no cards like this before.
The OG 7 mana 1/1.
Shifting Scroll is also identical to a card idea I've had for quite a while, except it was Epic instead of Common, costed 1 and could transform into any spell, not just Mage spells.

I planned to make a card very similar to The Darkness as the final boss card for an adventure (I only create the cards for an adventure/expansion once everything is written down), except the pieces would have been shuffled into your deck and not the opponent's, but now I have to change it.
I made some Rogue secrets too, but mine cost (0) which tbh makes more sense for rogue...

Haha! there is one! Very similiar to Saronite Chain Gang, except the deathrattle ;)
(proof that i made it before: imgur.com/4EpggCM)

Here's a few. I made all of these like a year ago, lol.
Repel = Can't be targeted by Spells or Hero Powers.
I'm creating a new custom expansion: The Bitter Reaches! Check out the progress on my Imgur page! ( http://lemondude617.imgur.com/ )
I made this guy a while before Knights of the Frozen Throne.
Spectral Pillager
Got another one I didn't realize before. It's all correct except the mana cost.
Snowfury Giant
Check out my Hearthstone expansion: The Shifting Sands
I made Dark Conviction that was one mana cheaper for the Expansion competition. Only thing wrong was mana cost and name(Obviously).
Edit: I also made this. Similar to Ice Fishing
I want a new title, but Flux won't let me have one,
I made this for a Custom Class, the exact effect, cost and text of Devolve.
Also the main theme of the same class, predicting the Grimy Goons, though I greatly understimated the effect.
Could be said as a more expensive version of Living Mana, though EVEN more risky.
Venomstrike Trap with a small twist. Obliterate but as a class card for Warlock and Lunar Visions but with spells.
The joke is you.
A bunch of cards I made before WOG:
Ticking Abomination lite?
Tuskarr Fisherman that activates more than once and doesn't require a friendly minion.
Sated Threshadon lite?
Sort of a Dead Man's Hand.
Okay this isn't similar to any real card (Ice Rager? Shadow Rager?); I just want Blizzard to make this lol
So far I've come close with:
Pretrified Guardian created prior to LoE Naga Sea Witch
Arcane Volley created prior to MSoG Greater Arcane Missiles
Ignite created prior to Un'Goro's Grievous Bite
Defender of Quel'Thalas created prior to MSoG's Lotus Assassin
Chucky "Ten Thumbs", Hugely overvalued, but created prior to KotFT's Dead Man's Hand
Check out my entrant for the WCDC 9.12: Bilgewater Highroller
My card is even cuter than Snowflipper Penguin!
Custom cards :
CLASSES : Alchemist (CCC#5 | Phase V) | Chef (CCC#4)
EXPANSIONS : Year of the Scorpion (Year Comp)
Just thought I'd revive this thread because the new warrior weapon Bladed Gauntlet is practically identical to a card I made a while ago.
Check out my Hearthstone expansion: The Shifting Sands
The Paladin Legendary in this set
And
Lynessa Sunsorrow

I made Spirit Cow for my Darkmoon Faire Adventure (AKA the source of my Grand Adventurer flair). It was by far the most discussed card of the set.
I literally made this hours before they released Kobold Monk

Here are mine, between a spoiler because there are a lot of them (none are exact maches though):
Call to Arms. Mine is a Hunter card so it HAD to be worse.
Psychic Scream. Cyclone was the ultimate Druid of the Talon ability in WC3.
Cavern Shinyfinder
Scorp-o-matic
Explosive Rune
Sleepy Dragon
Doesn't correspond to any particular card, just to show that I tried to build a "if your deck has no minions" Hunter too.
Cursed Disciple, plus the Elemental tag. It was supposed to be a joke... Blizzard pls
Dire Mole, minus the Beast tag which I considered would be too good...
Wax Elemental
In addition to those:
