Blizzard seems to have included a limit on DEFILE, wich stops casting itself again after a short number of 5 interactions even if a minion has died the the last time it was cast.
So you're averaging 1.5 Damage per cast of Defile, and with the 30 cast limit, it's a total of 45 damage out of the combo, not counting the juggles from the setup.
Basically, you've created a faster C'Thun that also completely nukes the opponent's side of the board (assuming they survive it), in a ZooLock style deck.

Well I managed to survive with druid at 30 hp plus 7 armor and a couple decent sized minions on board. Killed him the next turn. So I guess it works, but there's so much setup that it gives the opponent a huge advantage....

Yeah, I"m not sure if the animations are the issue, or if the combo hit the limit, but it's not as efficient as some of the other OTK combos involving defile. That and Grim Patrons are probably are not fast enough for use with Defile.
Steps 3 and 4 can be swapped for one more juggle, but it takes advantage of Summoning Portal and Emperor Thaurissan causing all cards less than 3 mana to goto zero mana, so it's slightly more mana efficient allowing you a theoretical turn 6 OTK. By the way, this combo does not work in Standard because it's too much mana without the Emperor Thaurissan tick.
Once the first couple of Defiles go off and wipe your board except for the Knife Juggler and Necrotic Geist, each cast of Defile will do 2 damage to everyone on the board, resulting in 5 juggles per cast for 5 damage, most of which will (very quickly) be going straight to the face - which is significantly more damage than what can be achieved with Grim Patrons and Defile.
In any case, here's a very rough draft of a deck that allows you to get the combo off:

And to finally answer the question on how often Defile actually repeats itself, i can confirm the loop ends after 14 times. But I did count 'em manually, so feel free to recount. Here's my video of the loop breaking and failing to kill a 30hp mage. The knifes (of one single Knife Juggler) did a total of 24 damage (20 face, 4 to minions).
So just tested the Knife JugglerDeathspeakerGrim PatronDefile, and the OTK will do roughly 25ish damage because it is animation bound.
However, you may just win the match if you can make it go off because it is that annoying XD

There's one OTK that can work... but it has **A LOT** of moving parts and requires a lot of setup, specifically:

The combo works fine, I just died to grim patrons lock, 30 hp, some armor and my side of the board. As a reno mage.
(i threw the villager in to see where/ir the combo resets, so I could tell if the second Defile activates properly after the first one)
