If you make it the the second phase as the Medivh hero, Medivh the character comes out and comments on how you must be him from the future. Malchaezzar thinks you're in costume :D
Took me a few tries, but when I won, it was with a 1/1 Kel''Thuzad spawned by Barnes. I instantly used Reincarnate on him, and by the end of the turn, I had TWO 6/8 K'Ts at my command... for the cost of 6 mana. Game & match decided on turn 6 =).
Here is cheap deck that worked for me. The key was to survive until you have 2 call of the wild, so you can get good card from Mediv's staff
what good is 2 call of the wilds if the cunt just blasts the board clear with back to back 4dmg spread across 3 heroes? Jesus fucking h christ just when you think Ben Brode and team can rip out the soul of gaming any further.

This is the deck that I finally got it with. Over 8 hours of play time alone on that heroic. I am glad I waited a round, before I played KT and AS and had a bogkeeper up. This gave me 2 KT's, and a bog keeper for start of second phase and then it was GG. Thanks!!
Eventually I got him with a modified Reno Highlander Warlock. Copied the deck from some YouTube guy, but I don't remember the exact deck list any more.
Use a mage deck and put in 2 Animated Armor cards.... Those saved me! Try to set up counter spell before you kill Nazra Wildaxe. (mad scientist is a big help with this) Whatever minions you have on the board will stay as counter spell will take care of Prince Malchezaar twisting nether. AI went face and ignored my animated armor cards, which allowed me to only take one damage at a time.
I have a problem with the theory that counterspelling twister nether with a board full of creeps is 'enough' to win. No one mentions the fact that Cunthezaar can and will use another spell (not sure of the name) but it basically deals 4 fucking dmg to 3 different minions/enemies. Which will annihilate your board and then he armours for +10 and hero powers 2 6/6's in the same turn. How is that fair. Is this supposed to be fun? Talk about ripping the soul out of gaming Ben Brode.

II went with Shaman and Kel'Thuzad with Ancestral Spirit (a somewhat modified http://www.hearthhead.com/deck=175550/heroic-free-medivh-kelthuzad-shaman-2nd-attempt-victory), so that after the boss switch and Twisting Nether you end up with a board full of 6/8 Kel'Thuzads and whatever you've managed to play earlier.

This is the deck I came up with for the fight:
After coming on here, it looks like people had similar ideas, although it doesn't look like anyone else combined a Murloc deck with Counterspell.

Just beat it with reincarnate Shaman
took quite a few tries :/
After losing eight times with Anyfin Paladin, I crafted this deck and beat it in two tries.
Took me 1 attempt with this deck. http://www.hearthpwn.com/decks/626038-heroic-free-medivh-no-epic-and-legendary

I took some inspiration from the Anyfin and KT decks doing well and decided to combine them. Made this:
Gonna leave it here in case anyone wants to try it out.
This is my deck: Shudderwoc’Thun
