I was playing as Druid with the death knight active, my opponent was secret Mage. He had 1 card in hand + next turns draw coming. I was at 13 health. I choose to use my hero power to push 3 face damage instead of armoring up because it set up lethal for next turn, even though if I just armor up it's nearly impossible to lose from that position. Turns out his card is fireball and his top deck is fireball and I'm dead.
It is remarkable that you posted this here and not in the salt thread.
Played an elemental mage against a PW, and I was getting low on hp. I played a water elemental, and he wrongfully decided not to kill it. Instead he played two minions where one of them was a taunt. All I had to to do was to kill off his taunt, attack his face with the water elemental and control the game.
I thought carefully about my trades and accounted for mortal strike etc and was sure he couldn't kill me if I traded properly. Well, first thing I did was to trade my water elemental into his 3/3 taunt and thus couldn't freeze the hero. He proceded to kill me with his his arcanite reaper + 2x heroic strike on the following turn.
Lately all my miss-plays have been Defile related. There's no shortage of new ways for me to miss-play that card.
Edit: I just remembered a good one. I adapted my murlocs before casting Blessing of Kings and chose the make them immune to spells and hero powers option, robbing me of lethal, and allowing my opponent to win.
Playing with Druid against Quest Priest. He's at 15 health and has no taunts, I have 12 attack worth of minions on board, plus a Swipe in hand. Easy winning play, no? But the little genius in me thinks: "Hey, I'm not at level 60 yet with Druid, every little bit of experience helps, so I'll throw in a hero power before killing him". Only after HP comes the realization I had just 5 mana crystals (having played Living Mana earlier), and the lethal chance had gone :(
I bring him down to 2 health anyway... next turn he plays 2 DR minions, completing the quest, drops Amara, and eventually wins the game.
After this game I placed a big "Always check for lethal!" sign right below my monitor ;)

His turn started. Arcane intellect. I thought "ok i've won"... Fireball+frostbolt. Similar happened the same day. Now i was playing Corol Mage vs Evolve Shaman. He had no card in hand and no minions. I got alextrasa in my hand (it was the 8th turn) and 5 hp so i would win the next turn. Yes... of course... HP air wrath totem jade lightning

Dropping down my Faerie Dragon closely followed by my Blackwing Corruptor only to realise the faerie dragon was my only dragon in hand...
Had a game yesterday as Handlock where I did the good old use The Coin for Mountain Giant, leaving the giant at the same cost.
My excuse? Errrr...I was semi-watching TV and playing, haha.

Druid vs. Hunter.
I'm at 2 HP, I have lethal on board.
I decide to relieve some stress by playing out the classic druid combo. (Force of Nature + Savage Roar)
Forgot that one of my minions is a Knife Juggler. It juggles one of my opponent's minions which happens to be a Leper Gnome.
The rest is history.
No matter how many times I do it, I still apparently fail to remember not to play the coin before trying play my giants... Doh!
Not really a misplay, but the other day I made a deck for fun using my two golden Renounce Darkness, for the extra bling...
...played 5 games without drawing Renounce, thinking "bloody hell, what's with my luck today?"

i played for like 4 games a "inner fire priest" without inner fire (forgot to add it to my deck) or any kind of way to buff the attack of my minions.
i thought i was being really unlucky
