Have you ever played the wrong card because it looked like the card you wanted to play?
Today in Arena I switched my Puddlestomper's stats with Kooky Chemist instead of healing it with Hozen Healer - both bald guys. Once I played Divine Favor early in a game against Reno Priest instead of a golden Truesilver Champion - the wrong yellow card. It usually happens when my eyes are tired from playing for a long time or I'm so intent on the board that I don't carefully check my hand.
Playing Rez priest...deck is built around Resurrect and Onyx Bishop..I needed to get something on the board against an aggro deck..so on turn 4 I inexplicably coined Onyx Bishop despite having no minions prior to that...I was sad to realize it was my similarly statted Barnes..
I made a Paladin deck in Wild that had 1 Coghammer and 1 Rallying Blade because I wanted to test and see which one fit the deck better.
I played the wrong one like two games in a row. I realize that the cards have different art, but on a mobile device all you really see is the mana cost on the top of the card.
Around I've just started hearthstone, I played Cold Blood instead of Eviscerate buffing the enemy minion instead of killing it giving him lethal next turn. I know they don't look a like and even cost the same but I had both of them in my hand I was so sure I was killing the minion
Not so much playing the wrong card, but one thing I do all the frickin' time is forget to add spell damage. Can't tell you how many board clears and lethal strikes I've missed because I forgot I had spell damage. I wouldn't be so frustrated at myself, but the spells even sparkle when they have spell damage. No excuse for me forgetting, yet I do it all the damn time.
Usually I'll chump a couple 1/1's into various minions to prep for a Hellfire or whatever, and not realize that I just wasted damage until it's too late. Another one that kills me is when I actually realize I have spell damage, but calculate it for multiple spells and not realize my Drake is going to die after the first spell. bleh

Probably. Most of us don't remember it because the only time that happens is when you're playing drunk or high.

My mandibles which are capable of pressing down and tearing, my talons which are known to intercept and hold.
In Wild I got a stealthed 1/1 from a Piloted Shredder and was sure I could kill the enemy's big minion with Patient Assassin . Was disappointed when I just got a Jade Golem from a Jade Swarmer after suiciding it.
I confuse Wild Pyromancer with Doomsayer all the time.

I did this with Renolock, played Dirty Rat before Brann, no fucking clue why

The Coin and Circle of Healing back then when control priest with Auchenai Soulpriest was viable ^^ They look kinda similar :)
